Does anyone have a sample Due Diligence report or template that they'd be willing to share?

Also, do you have any specific triggers, questions, or liabilities that you ALWAYS look out for or need answered throughout DD. I heard a good one the other day, ask the owners if there are any undocumented liabilities or promises that have been made to anyone that the buyers should know about. 

I have a comprehensive DD question list already. I am more interested in people's tricks (and sample or template DD report) , if they are willing to share.
