Team Building Activities for Company Offsite?

Looking for suggestions on team building activities for a company offsite. Anyone have any resources / suggestions? Offsite is in Las Vegas. Also, any suggestions / introductions to key note speakers that are reasonably priced would be appreciated!

I like to decide what the issue is that I am addressing and then build an activity that will assist it. Examples might be:

* Do the team members just need to get to know each other in a formal setting to build rapport and just have fun together after a tough time?
* Do you have a few team members who seem to cause a fire drill in the organization every couple of weeks
* Are folks resistant to change/trying new things, etc. Or, are they chasing every shiny object?
* Are there cultural differences as you merge two teams into a functioning whole?
