Hi all - I am searching for a small medical billing / revenue cycle management (RCM) company to acquire. We are looking to acquire ASAP and can move quickly. Investment Criteria: Sector: Medical Billing / RCM - Specialty agnostic, but prefer Post-Acute / Home Health, Ortho, Derm, Behavioral Health). Financials: $250k – $1.5M EBITDA Geography: Nationwide (Remote preferred). Model: Recurring revenue, low churn, low customer concentration Appreciate any connections in the space and happy to provide any other info that may be helpful.

I’ve spent some time exploring the RCM space, I have BPO experience and thought that could work well as a new owner. One of my professors was an investor in a large RCM company, which gave me a helpful lens into how the industry operates. That said, I ran into consistent challenges pursuing deals in this sector, though others may have had different experiences 1. Industry experience is heavily weighted: In my experience, sellers and brokers place a strong premium on prior RCM operating experience. There’s a clear perception that the business is highly nuanced and difficult for outsiders to step into. Despite having capital lined up, I often found it difficult to be taken seriously without that background. 2. Valuations can be difficult to finance: Even smaller deals tend to command relatively high multiples. Sub-$1M EBITDA businesses were often priced over 5x and in one case, I saw 7x. At current interest rates, that can be challenging from an SBA financing perspective, where lenders are typically more conservative on leverage at those levels. There are certainly ways to navigate both of these hurdles, but for me, the friction was high enough that I decided to focus my search elsewhere.
