David - I provide consulting for equity all the time. You have to carefully structure because there are significant tax consequences. Keep that in the front and back of your mind when you enter into such agreements and consult with professionals who can help you do it optimally. Good luck!

I can send you an advisor equity agreement, but are you sure that's what you're looking for? I think this is usually done via a rollover agreement along with an employment agreement. Would suggest you talk to your counsel before sending draft agreements.
