Acquisition Transition Template?

I seem to have lost my 'go-to' excel acquisition implementation template that covers all the various aspects an acquisition transition. Something broken out by functional area (i.e. Finance, HR, Ops, Marketing, Sales, etc.). Not sure if anyone has anything they can share that would resemble this, but appreciate any help in advance!
